This article explains how to create a new SDLT Form in Unity® Practice Management. 

Accessing the Forms Library

1. Click Forms, and select Library to access the Forms Library

   

How to Create a New SDLT Form 

1. Click New SDLT Submission from the Forms Library screen

   

2. Select a Case by searching for it or selecting from Recent Cases
3. Select a contact for a:
   • Vendor
   • Vendor Agent
   • Purchase
4. Once done, click Proceed
